Update for mtimes (and a better API).

Specifically, lookup and stat results for plain files now carry an
mtime as well, and qfs_s_lookup_result() is gone, replaced with
qfs_s_lookup_result_{dir,file,link}, so as to get compile-time checking
of arguments and types.
