Rework for changes elsewhere.

The biggest single change is a paradigm switch make it easy to use unix
or qfs as fstree root.  Some small but important bugfixes.  Eliminate
some unused code.  Probably others; the paradigm change turned out to
be intrusive enough that the diffs are extensive.
